Cardboard snack trays not cleaned: Lebanon County restaurant inspections, Jan. 22, 2024

The Pennsylvania Department of Agriculture, 866-366-3723, uses a risk-based inspection reporting process for restaurants and other food handlers.

Lebanon High School, 1000 S. 8th St. Lebanon, January 12. Pass. No violations.

Southeast Elementary Cafeteria, 499 E. Pershing Ave. Lebanon, January 12. Pass. No violations.

Southwest Elementary School, 1500 Woodland St. Lebanon, January 12. Pass. One spray bottle, a working container of sanitizer taken from bulk supplies, had previously been labeled with the common name of the chemical; however, the writing on the bottle had worn off. Corrected.

Stop and Chill, 259 S. 6th St. Lebanon, January 12. Pass. Gravy, Mac & Cheese, mashed potatoes, and beans, were held at 115°F, in the food warmer, rather than 135°F or above as required. This violation was observed on 1/30/23. Commercially processed food that has not been heated previously, needs to be heated to 135°F prior to be placed in the hot-holding unit. The piece of equipment used for hot-holding in this facility is designed for holding food hot; it is not intended to be used for heating food. Dumpster lids were open at the time of the inspection. Lids need to remain closed. Paper towel dispenser empty at the handwash sink in the employee toilet room. There are items along the exterior rear wall of the facility that are considered rubbish and need to be removed from the property.

Health Check Juice Bar, 40 E. Main St. Annville, January 11. Pass. Observed deeply scored cutting boards not resurfaced or discarded as required.

Hutter’s Gas & Shop, 1041 Maple St. Lebanon, January 11. Pass. The interior of the ice machine, a food contact surface, was observed to have black residue and was not clean to sight and touch.

Lebanon Middle School Cafeteria, 350 N. 8th St. Lebanon, January 11. Pass. The interior of the ice machine, a food contact surface, was observed to have a discolored residue dripping along the interior side panel.

Northwest Elementary Cafeteria, 1315 Old Forge Rd. Lebanon, January 11. Pass. The fan covers in the walk-in cooler, non-food contact surfaces, are starting to accumulate a small amount of dust/debris.

Allen Theatre and Back Stage Cafe, 36 E. Main St. Annville, January 10. Pass. No violations.

Franz’s Tavern, 1400 S. 5th Ave. Lebanon, January 10. Pass. Food employees observed in the kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid.

Penn Corner, 45 S. 9th St. Lebanon, January 10. Pass. Cardboard snack trays, non-food contact surfaces, are not cleaned at a frequency to preclude accumulation of dirt and soil. This issue was observed during the inspection one year ago. The front door to the outside, located in the front of the food facility is being propped open. Corrected. Toilet tissue is not on the dispenser.

Fresh Bubble Tea, 1788 Quentin Rd. Lebanon, January 9. Pass. Milk was held at 48°F, in the refrigerator, rather than 41°F or below as required. Warewashing sink does not have basins large enough to accommodate the largest piece of equipment needing manual warewashing. Food facility does not employ a certified food employee as required.

Plaza Azteca, 200 Garden Ave. Myerstown, January 9. Pass. Ice machines not cleaned at a frequency to prevent the presence of mold inside. Observed clean pans in dish area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). The light intensity in the third floor food preparation area is not at least 50 foot candles.

Subway of Cleona, 443 W. Penn Ave. Lebanon, January 9. Pass. Observed a deeply scored cutting board not resurfaced or discarded as required. Food facility does not employ a certified food employee as required.

Twin Kiss, 361 W. Lincoln Ave. Myerstown, January 9. Pass. Food employees observed in prep area, not wearing proper beard covers. Assorted containers on drying shelf were observed to have food residue and were not clean to sight and touch. Removed for cleaning.

Wal-Mart Supercenter #2023, 1355 E. Lehman St. Lebanon, Complaint, January 9. Pass. No violations.
